The twisted cartooniness of this image fits perfectly with the realistic textures employed as a background. This guy looks like the sort of character you would look at perfectly normally if you met him out on the street, minus the creepy syringe and tophat, and without that insane eyebrow thing. Inserted into the proper environment, however, and he transforms into this Dr. Salmon, a perfect reflection of everything you DON'T want in a medical operator. Seriously, who would expect someone with a stache like that to be evil? NOT me! Well, maybe if he had that eyebrow thing going on....

at any rate, the style you achieved here melds perfectly the painted figure and the realistic elements. For instance, was that syringe made in a 3d program or drawn? I find myself wondering which.
